Carer Video — Log a safeguarding concern

Read these lines aloud, one at a time, pausing naturally. Each amber PAUSE is a natural stopping point. Don't memorise — just read.

📖 Read-along version

Hi, I'm Clive from SafeguardingUK.

In this video I'll get you signed in, then show you how to log a safeguarding concern.

It's the main thing you'll do.

Use it whenever something worries you about a person you support — big or small.

There's no training needed — if you can use a phone, you can use this.

Pause

First, let's get you signed in.

Your manager will send you a welcome email, with a link inside.

Open it, and tap that link.

It takes you straight to a screen to choose your own password.

There's no temporary password to type in.

Enter your new password, and you're in.

You only do this once. After that, just sign in with your email and the password you chose.

Pause

At the top of the screen, tap Report Concern.

Choose Service User Concern — that's about a person you care for.

Or choose General Concern — that's anything else.

For this video, we'll use Service User Concern.

Pause

Pick the person from your list.

These are the service users assigned to you.

Can't see them? You can search by name.

If no one's there, contact your manager — they'll add them for you.

Pause

Now choose the category — the kind of concern it is.

If a "Specific type" box appears, pick the closest one there too.

Pause

Next, choose "How you know".

That's things like suspected, alleged, witnessed, or disclosed.

Just pick the one that fits.

Pause

Then choose the Severity — how serious it is.

Not sure? Pick the closest. The person dealing with it can change it.

Pause

Say where it happened, and when.

Pause

Now describe what you saw or heard.

Stick to the facts, in your own words. Don't worry about spelling.

Add any immediate action you took. Did you call anyone? Did you move the person? That goes here.

Pause

Next: who do you want this concern allocated to?

Type the person who should pick it up and deal with it.

That's usually your manager or care coordinator.

Not sure? Just leave it blank.

Pause

You can add photos if they help — that's optional.

We'll skip the photos for this video.

Then tap Submit Concern. Done.

If in doubt, report it. It's always better to log a concern right away.

Pause

That's how you log a concern.

Thank you for watching.
